 UHC Day 2024 Theme: Health: it's on the government!

When populations are healthy, communities and economies prosper. That is why this Universal Health Coverage Day 2024 we call on decision makers around the world to protect people from financial hardship when accessing health services. 

It is the responsibility of governments to ensure that healthcare is always affordable or free, particularly for the poorest and most vulnerable among us. 

Background : Each year on 12 December, the world celebrates International Universal Health Coverage Day (UHC Day), an official UN-designated day, to raise awareness of the need for strong, equitable and resilient health systems and universal health coverage (UHC). 

UHC Day marks the anniversary of the UN’s historic and unanimous endorsement of UHC in 2012 as an essential priority for international development. Since then, it has become the annual rallying point for the growing movement for health for all. 

Every 12 December (12.12), advocates raise their voices to share the stories of the millions of people still waiting for health, to call on leaders to make smarter investments in health and to remind the world the imperative of Health for All.

The high cost of health services is the primary reason people miss out on essential health care. These costs often force families to choose between paying for medicines or a visit to the doctor and other necessities, such as food, housing and education.

Health costs and illness should never push people into poverty! Investing in universal health coverage and financial protection not only enhances health and well-being but also fosters greater equity and social cohesion.

Everyone should have access to an affordable core package of essential health services, including quality medicines. No-one should be pushed into poverty when accessing these services.

Good Health Practices Start Young

A healthy body is the best start for children who are studying at school, as it helps them to learn more effectively and gives them the best start in life. A good education leads to more opportunities and a better income as the child becomes an adult, and good personal health allows children to flourish in the classroom.

By providing medical examinations for students on entry to the school and educated observation by the class teacher, the Indian School Health Committee aims to prevent and cure illnesses and disease that can hinder a child scholastically. However, this scheme has so far failed to bring about successful results due to the lack of follow up checks, funding issues in schools, and ineffective co-operation between local authorities, schools and parents. That’s why the Trinity Care Foundation has introduced school heath programs. 

Simple solutions

Some of the problems suffered by young students in Indian schools are relatively simple to treat or could be prevented through better health education.

Anaemia due to poor diet is a common health problem which can be both preventable and easy to treat in children. Symptoms of anaemia in children include weakness, fatigue, headaches and irritability. It is known to affect performance at school, causing a decrease in attention span and even learning difficulties.

Fortunately, anaemia can usually be treated by taking iron tablets on a daily basis, or alternatively through a healthy diet. Iron-rich foods can combat anaemia; these include include chicken and meat – in particular liver – as well as beans, lentils, eggs and fish.

In terms of dental hygiene, with regular brushing of teeth and a healthy diet, children are less likely to have cavities. Unfortunately, there are children who may have never seen a dentist and attempt to live life with rotten teeth. 

Bad teeth can cause more problems than you might have first realised. Obviously, there is the level of pain a child has to suffer through – but take into account attempting to learn or do an exam with toothache. The constant pain can cause children to perform at a standard lower than what they are capable of, which sets them at a disadvantage even so early in life.

Through school health programmes, problems such as these can be spotted early – usually by the class teacher – and the effects on learning and a child’s future minimised.

Health into adulthood

As children grow into adults, they will carry lessons in health with them, allowing them to benefit from a happier and more fulfilled life. However, it is also important to ensure parents and other adults in the community are educated in a similar way. Reinforcing healthy living at home encourages these habits in the next generation.

Community outreach programs bring health education and screening into the heart of a community, allowing adults to benefit from these important life tools. Trinity Care Foundation takes quality healthcare direct to underprivileged communities, allowing them to benefit from it. 

The community outreach program provides a wealth of health screening solutions – from dentistry to Gynaecology – and offers medicines free of charge. This is helping to bridge the gap between the rich and poor in Indian society, preventing needless deaths by providing easy to access treatment for diseases, as well as education regarding common illnesses.

Child to adult – a journey in good health

When good health practices are observed by the entire community, it provides a wealth of benefits. Children who are more alert in school can perform better – and a better education provides a route to employment and income, which is ideal for deprived areas. 

Understanding the nutrients that a body requires to stay active and healthy leads to a better diet, which is beneficial for preventing a range of different health problems. A community of healthy adults poses a lesser strain on local hospitals and healthcare providers, meaning they can dedicate more time and money to curing and treating difficult illnesses.

Additionally, a community in better health is stronger, more active and more productive. This is especially useful in rural villages where farming is the main source of work. People in areas like this often don’t have access to a doctor, dentist or hospital – but they depend on their bodies to be able to work in the fields and bring in an income. Preventative healthcare such as that brought through outreach health programs, allows access to doctors and dentists to resolve issues before they become serious, and even fatal problems.

Mobile Health Camp at Somanathapura Village

On the 9th of December 2012, Trinity Care Foundation conducted a Free Mobile Health camp in Somanathapura Village Government School in Bagepalli Taluk, Chickaballapura District, Karnataka, India. 

This medical camp aimed at providing basic healthcare services focusing on Orthopedics, Gynaecology, Dermatology, Diabetology, Cardiology, Dentistry, Pediatrics & Ophthalmology. Basic Medicines were provided free of cost.

The Organization worked in conjunction with government health officials, Gram Panchayath Members and Hindustan Aeronautics Limited, HAL members in order to organize this event.

Health Workers from villages were directed in advance to escort patients to this camp. In order to spread the word about the camp, the foundation organized press releases in local newspapers and also put up advertisement posters in and around the village. Additionally local help was taken to cover the event.

A team of 36, including 18 Doctors as well as observers from HAL, Bangalore, set out from Bangalore early in the morning and arrived at Somanathapura Government School, the site of the camp at around 10:00 am. The Village is 148 Kilometers from Bangalore City towards Devanahalli.

Once there, the team began examining patients – who consisted mostly of young children and Adults – to determine the extent of their problems and decide on a possible course of action to correct these problems. Patients were examined by our team of doctors. 

The team distributed Medicines, conducted GRBS Tests and provided Dental Treatments on the Mobile Dental Unit. Around Six Hundred people were examined for various problems. The doctors explained to each patient – and in many cases, parents and other relatives of these patients as well – exactly what procedures needed to be done to cure them. While the team had anticipated a problem in convincing patients of the need for surgery, there did not appear to be any such issue. 

Each person examined was given a diagnosis and a prescription sheet, and some patients were also asked to come into Bangalore for surgeries on a given date.

The camp closed at 3:30 pm, after every single patient had been observed. Fluoride is the main cause in Bagepalli Taluk. Problems caused by fluoride include Dental Fluorosis - a disease of the teeth, weakening of bones and bone loss, bone cancer, kidney problems and hormone disruption.

Patients who require surgery will be treated at hospitals in Bangalore in the near future.

Trinity Care Foundation hopes to build on its successes by organizing such camps soon. This Mobile Health Camp was conducted for Hindustan Aeronautics Limited (HAL), Bangalore as part of their Corporate Social Responsibility (CSR) Initiative focusing on Community Health.

Healthy States, Progressive India

NITI Aayog has been mandated with transforming India by exercising thought leadership and by invoking the instruments of co-operative and competitive federalism, focussing the attention of the State Governments and Union Ministries on achieving outcomes. As the nodal agency responsible for charting India’s quest for attaining the commitments under the Sustainable Development Goals (SDGs), it was necessary to devise a mechanism for measuring outcomes particularly in the critical social sectors – such as Health and Education, where India’s record has been less than stellar. This was intended to provide feedback to all stakeholders as to whether we are on course to what we have set out to achieve, and deviations, if any, to be pointed out in time to  ensure necessary mid-course correction.

It is important to realize that implementation of social sector programs is squarely in the domain of the State Governments and India’s achievement of SDGs is therefore critically dependent on the action in the States. Nudging States towards improving their social outcomes therefore requires developing indices that would capture annual increments in performance through an independent third party process and publish these. It is true that summarizing the complexities of a given sector and condensing it in an Index has its own limitations. However, in an environment where the focus is on budget spends and outputs with limited attention on outcomes, there is a need to increase competition among States to encourage them to strive evermore for increasing the pace of change.

The Health of its population is central to a nation’s well-being and productivity. While India has made some significant gains in improving life expectancy and reducing infant and maternal mortality, our rates of improvement have been inadequate as a nation.

Further, there are large variations in health system performance and outcomes achieved across States. The “Performance in Health Outcomes” Index seeks to capture the annual progress of States and Union Territories (UTs) on a variety of indicators – Outcomes, Governance and Processes. While we have also reported the overall levels of performance of States, the focus of the NITI Index is to propel change, highlighting those States that have shown most improvement. The exercise has been spearheaded by NITI Aayog in collaboration with the Ministry of Health and Family Welfare, with technical assistance from the World Bank, the authors of this report on the ranks and their interpretation.

The exercise, which is the first of its kind attempted by the Union Government was conducted over a period of eighteen months. In addition to the technical expertise of the World Bank, experts in public health, economics, statistics and health systems were consulted in the development of the Index. It involved extensive engagement with the States for finalization of the indicators, sensitization workshops for sharing the methodology, process of data submission and addressing concerns; mentoring of States for the data submission process on an online portal and independent data validation.

The process of Index development and implementation highlighted the large gaps in data availability on health outcomes.The need for making outcome data available for smaller states, more frequent and updated outcomes for non-communicable diseases and financial protection, and the need for robust programmatic data that can be used for continuous monitoring, were important issues that despite our efforts, could not be addressed optimally in this first round. Despite these challenges and limitations, it was decided to launch the Index in the first year as a model to measuring performance and ranking States on change. We thereby hope to spur action on several fronts in bringing about national level transformation. We will strive to address the lessons learned in this first round and refine the Index in the successive years of its implementation. The linking of the Health Index with incentives under the National Health Mission by the Ministry of Health and Family Welfare underlines the importance of such an exercise. It re-emphasizes the move towards performance based financing for better outcomes.

Why the need for Vision screening in Schools ?

Vision problem affects 13.5 million children and almost quarter of adolescents overall 12-17% children are reported to have eye vision problems. Poor vision in children affects performance in school or at work and has definite negative influence on the future of the child. Myopic children have trouble reading the black board notes or digital character of the class room presentation material displayed on screening.

School screening for vision in children is very essential and need to be made mandatory. Perhaps teachers and parents can help children who may be struggling with few vision conditions like squinting narrowing of eyes, headache, turning of eyes inwards, outward blurred vision, holding books too close all these need vision screening.

Children with blindness suffer long years of disability which severely impacts their overall development and restrains them from any meaningful participation in social and economic activities. Uncorrected Refractive Error (URE) is one of the major reasons for visual impairment among children, especially in the age group of 5-15 years, with Myopia leading the list of refractive errors.

Children usually don't complain of defective vision especially if only one eye is involved. They may not even be aware of their problem. They adjust to the poor eyesight by sitting near the blackboard, holding the book closer to their eyes, squeezing the eyes and even avoiding work requiring visual concentration. This evades early detection. Timely detection of these problems and their correction by spectacles can tremendously improve the child's potential during his formative years. Visual impairment is an avoidable burden in the life of a child, his family and the society. Periodic screening of all children helps in early identification of visual defects and correction of REs, by prescription/use of corrective spectacles.

 In India it is estimated that 2-3 % of school children in rural areas have refractive error. In the urban areas the proportion is even higher. One study shows that 5.1% of children in schools had visual acuity of <6/12 in the better eye.

Ensure eye care for all

WHO South-East Asia has 140 million visually impaired people. The Region has an estimated 12 million blind people, 8 million of them in India. More than 50% of all blindness is caused by cataract which accounts for 33% of visual impairment. Uncorrected refractive errors are responsible for 43% of the visual impairment in the Region. About 65% of the visually impaired people are 50 –year old and above.

These statistics should be viewed against the fact that 80% of visual impairment is avoidable. And that the blindness prevention strategies and restoration of sight are among the most cost-effective interventions in health care.

To reduce avoidable blindness and visual impairment, we must ensure universal access to comprehensive eye care services. There is a need to review and reinforce existing measures so that nobody suffers visual impairment and those with unavoidable vision loss can still achieve their full potential. World Sight Day on 8 October will focus on ‘eye care for all’.

The WHO Global Action Plan (GAP) for the Prevention of Avoidable Blindness and Visual Impairment 2014-2019, emphasises on improving eye health for everyone and reducing the prevalence of avoidable blindness and visual impairment by 25% by the year 2019.

Implementing simple measures such as eye care services in schools can help detect and correct refractive errors in children. And quality surgical procedures can help cataract patients get back to normal lifestyle very quickly.

School Health Internship

Trinity Care Foundation calls for female interns who are genuinely interested to travel, learn and work in Government Schools in Bangalore,Karnataka.

The internship offers a rare opportunity to work hands-on in schools, learn from pubic health professionals and help those in need of medical attention. The work will focus on issues pertaining to school students and cover health check-up, eye check-up, dental check-up, dental treatment and health education. The internship is for 1 month. 

Eligibility

The internship is open to Doctor, Dentists and Students [ Medical & Dental ] of Indian origin. Ability to converse in the local language is a must. Above all, we welcome those with curiosity to explore and humility to learn.

Location: Bangalore,Karnataka
Languages: For those interested to work, ability to converse in Kannada is required.
Duration: Nov 18-Dec 18, 2015

Type of work

The team will cover a minimum of 15 government schools in outskirt's of Bangalore, and undertake the following activities:

• Pre-screening of students for overall health, eye-care,CHD, dental caries, oral lesions, facial deformities, etc., 
• Facilitating medical intervention for those shortlisted through the pre-screening
• Impart awareness on health issues, ill effects of tobacco and tooth brushing techniques to students of government schools.
• Undertake training for teachers and health workers to carry forward the work.

Orientation for interns : Selected interns for Karnataka will have to attend a day orientation program in Bengaluru on Nov 11.

Last date for application: Bangalore, Karnataka : Nov 9, 2015

We will organize for the intern’s travel and food during the period of internship.Certificates will be provided to those who have successfully completed the internship.
